2025-26 Transportation Request Form NOW OPEN!

Complete ONE FORM PER STUDENT for the 2025-2026 school year.Student transportation requests during the school year will receive an email with an
assigned  bus stop within 5-7 school days.

• To be completed by in-district*  K-12 students only requiring transportation.
• GSRP and ECSE student requests go through Model Elementary.
• Please ONLY complete if you are planning to use transportation.
• We are not accepting requests for students with two different stop locations.
• Each student will only be assigned one (1) stop location.
• Students are only permitted to ride the bus to and from their assigned stop location.

*No transportation is provided for School of Choice students.
